Service description

Working in conjunction with local housing providers, we are usually able to source accommodation with or without support packages tailored to individual needs, or just focused on setting up and maintaining a home. The goal of this service is to enable people to become more independent within their abilities. We aim to achieve this by working closely with the people we support and other stakeholders in learning and respecting their choices whilst engaging in activities that enhance their daily living skills. Based on individual strengths and capabilities, we engage individuals who require this support in sourcing a local accommodation where they wish to live. The long term focus is to align individual support provision with their capabilities as the level of independence increases.

Eligibility

This is a learning disability or mental health service for adults requiring housing with or without support within a single or cluster accommodation.
